NutriScan – Canine Food Sensitivity & Intolerance Test

1. Kit
Do you have the NutriScan Kit to perform the test? If not, you will need to order one to perform the test.

2. Complete Test Request & Order Form
NutriScan offers you the convenience of ordering the Kit and the Test online at the same time. Please print and place a copy of your completed online Test Request Form in the box when you return the sample to Hemopet.
If you choose to wait on the test until you receive the kit, you will need to complete the Test Request Form that will be included with the kit. For detailed instructions, please visit How to Test.
Another option is to come back online to complete and print the Test Request & Order Form.

Mailing Details
You can send the samples via FedEx, UPS or USPS Priority Mail. Return mailing to Hemopet needs to be in a protected package or envelope. No refrigeration is necessary, and the saliva sample is viable for up to 30 days; however we do recommend that the sample should arrive at Hemopet within 10 days of being taken.

International
For International Samples - including Canada and Mexico - US Customs clearance requires a letter specifying that the "sample is from a dog or cat for diagnostic purposes only, and is non-infectious." Please click here for full instructions.

Results
Allow about 2-3 weeks for the results.
